Postnatal Consultation
A lactation consultation normally includes visual and physical assessment of the baby’s mouth and the mother’s breasts, observation of breastfeeding, analysis of the breastfeeding history, demonstration of techniques to improve breastfeeding, and, where appropriate, the use of equipment.
I give permission for Lactation Consultant Mo Brownlee of Nurture Breastfeeding to carry out the above.
I understand that I have the right to decline any or all suggested techniques, equipment or actions.
I understand that all medical care is provided by my doctor and healthcare professionals. I give permission for information to be shared with them where appropriate. I understand that Mo Brownlee is required to share information with relevant professionals if there is concern regarding the welfare of either the client or baby.
Antenatal Consultation
An antenatal lactation consultation provides an opportunity to explore concerns, discuss how to optimise early breastfeeding, and develop a plan to help you feel informed, prepared and confident. Inspection of a mother’s breasts may assist this process.
I give permission for Lactation Consultant Mo Brownlee of Nurture Breastfeeding to carry out the above.

Record Keeping
I understand that a record will be completed during the consultation containing personal and medical information. This record will be securely maintained by Nurture Breastfeeding in accordance with the NHS Medical Records Code of Practice (Scotland) 2012. I understand that I may request access to this record at any time.
I consent to a record of my contact details being securely maintained in accordance with the General Data Protection Regulation (GDPR). I understand that this information is used solely by Nurture Breastfeeding and will not be disclosed to any third party except where legally required.
Please note that clinical records relating to the care of babies are required to be retained for 25 years in accordance with the NHS Code of Practice (Scotland) 2012.
Nurture Breastfeeding is registered with the Information Commissioner’s Office (ICO). All information is held securely and managed in accordance with applicable data protection legislation.

Cancellations
Cancellations made with less than 24 hours’ notice may incur a fee. If you have concerns regarding a scheduled appointment, please contact Mo as soon as possible to discuss your circumstances.
